After performing Achamana, clean your right hand palm by taking water into it through five-vessel. Take fresh water, Akshata, flowers etc. into cleaned right hand palm and read Puja Sankalpa Mantra.
Oṃ, That is the Supreme Truth (Tatsat).
In the sacred time of Brahma’s day, during the second quarter of the day,
in the Śveta–Vārāha Kalpa, in the Jambūdvīpa, in the land of Bhārata,
in the region (mention your country/place), in the village/town (mention your place),
of the gotra (mention your gotra), I, (mention your name),
in the first quarter of the Kali Yuga, during the month of Kārtika,
on the Trayodaśī Tithi (13th lunar day) of the Krishna Paksha (waning fortnight),
on this auspicious weekday (mention the day),
for pleasing Lord Dhanvantari,
and for the growth of health, prosperity, and wellbeing,
I perform this Śrī Dhanvantari Pūjana (worship of Lord Dhanvantari).
Take water in your right hand using the Panchapatra (five-vessel set) and sprinkle it on yourself for purification
ॐ अपवित्रः पवित्रो वा सर्वावस्थां गतोऽपि वा ।
यः स्मरेत् पुण्डरीकाक्षं स बाह्याभ्यन्तरः शुचिः ॥
PADACHEDA
ओम् अपवित्रः पवित्रः वा सर्व-अवस्थाम् गतः अपि वायो स्मरेत् पुण्डरीकाक्षम् स बाह्य-अभ्यन्तरः शुचिः ।

ENGLISH MEANING
Whether one is impure or pure, or in any state whatsoever,
the person who remembers Lord Vishnu (the lotus-eyed one)
becomes purified both externally and internally.
TRANSLITERATION
om apavitraḥ pavitraḥ vā sarva-avasthām gataḥ api vāyo smaret puṇḍarīkākṣam sa bāhya-abhyantaraḥ śuciḥ .

After the self-purification, begin Puja with the meditation of Lord Dhanvantari. Dhyana should be done in already ignited Ghee lamp flame kept in front of you.
I meditate upon Lord Dhanvantari —
the four-armed Lord, clothed in yellow garments,
resplendent with divine ornaments,
and worshipped by both Devas and Asuras.
I meditate upon the youthful, lotus-eyed Lord,
adorned with all celestial ornaments,
holding the pot of nectar (Amṛta) in His hand,
accompanied by the goddess of auspiciousness (Śrī).
I meditate upon that auspicious Lord Dhanvantari,
the divine enjoyer of Yajñas and offerings,
revered by Devas and Asuras alike,
who is clad in pure white garments.

caturbhujam pīta-vastram sarva-alaṅkāra-śobhitam dhyāyet dhanvantarim devam sura-asura-namaskṛtam ..1..
yuvānam puṇḍarīkākṣam sarva-ābharaṇa-bhūṣitam dadhānamamṛtasya eva kamaṇḍalum śriyā ayutam ..2..
yajña-bhoga-bhujam devam sura-asura-namaskṛtam dhyāyet dhanvantarim devam śveta-ambara-dharam śubham.

O Supreme Lord of the gods,
Radiant One, Lord of the Universe —
I humbly invite You to accept
this worship being performed by me,
O most excellent among the celestial beings!
I invoke and welcome Lord Dhanvantari into this sacred space.
After Shri Dhanvantari has been invoked, take five flowers in Anjali Mudra and leave them in front of the you to offer seat to Shri Dhanvantari while chanting the prescribed Mantra.
नाना रत्न-समायुक्तम् कर्त-स्वर-विभूषितमासनम् देवदेवेश प्रीति-अर्थम् प्रतिगृह्यताम् श्री धन्वन्तरि-देवाय आसन-अर्थे पञ्च-पुष्पाणि समर्पयामि।

O Lord of Lords, O Supreme Healer,
Please accept this sacred seat,
adorned with many precious gems
and embellished with divine beauty,
offered with devotion for Your pleasure.
I offer these five flowers to Lord Dhanvantari as a symbolic seat
पाद्यम् गृहाण देवेश सर्व-क्षेम-समर्थ भोः भक्त्या समर्पितम् देव लोकनाथ नमः अस्तु ते श्री धन्वन्तरि-देवाय पाद्यम् नमः।
ENGLISH MEANING
O Lord of Lords, accept this water offered for washing Your divine feet.
You are capable of granting all welfare and well-being.
This offering, made with devotion, I place before You,
O Lord of the worlds — my salutations unto You.
I offer this sacred water at the feet of Lord Dhanvantari.

pādyam gṛhāṇa deveśa sarva-kṣema-samartha bhoḥ bhaktyā samarpitam deva lokanātha namaḥ astu te śrī dhanvantari-devāya pādyam namaḥ.

Salutations to You, O Lord of Lords,
Salutations to You, the Upholder of the Earth,
Salutations to You, the Support of the Universe!
Please accept this sacred Arghya (offering of water) with devotion.
It is enriched with fragrance, flowers, rice grains, and sacred fruits.
Kindly receive this water, O Compassionate Supreme Lord,
as a token of love and reverence.
I offer this sacred, fragrant water to Lord Dhanvantari in divine reverence.

namaḥ te devadeveśa namaḥ te dharanīdhara namaḥ te jagat-ādhāra arghyaḥ ayam pratigṛhyatām gandha-puṣpa-akṣataiḥ yuktam phala-dravya-samanvitam gṛhāṇa toyamarghya-artham parameśvara vatsala-śrī dhanvantari-devāya arghyam svāhā.

Please accept this divine sandalwood paste fragrant, cooling, and most pleasing anointed with devotion as a sacred offering to You.
I lovingly offer this sandalwood paste to Lord Dhanvantari.

O Lord of the Universe!
I worship You with flowers of sevantika (chrysanthemum), vakula, champaka,
lotus petals, punnāga, jasmine, oleander, and mango blossoms along with bilva leaves, coral buds, tulasī leaves, and jasmine garlands.
I humbly offer these flowers to Lord Dhanvantari with devotion.
O Lord! This incense,
born from the essence of forest trees,
rich with divine fragrance and most pleasing,
is fit to be inhaled and enjoyed by all the gods.
Please accept this offering of incense, O Divine Healer!
I offer this sacred incense to Lord Dhanvantari.

O Lord of Lords! Please accept this sacred lamp,
kindled by me with ghee and wick, joined with divine fire —
a lamp that removes the darkness of all three worlds.
With devotion, I offer this light to You, O Supreme Soul.
Protect me from terrible suffering and the darkness of ignorance.
Kindly accept this offering of the lamp.
I offer this divine light to Lord Dhanvantari with reverence and devotion
O Lord Dhanvantari! Please accept this offering of food —
made of sugar, sweets, curd, milk, and ghee,
along with all varieties of nourishing, edible, and enjoyable dishes.
As per my capacity and devotion,
I offer this Naivedya unto You, the Supreme Healer.
Salutations to the vital airs of life —
Om Prāṇāya Svāhā (to the life-force),
Om Apānāya Svāhā (to the downward-moving energy),

I offer water for washing the hands,
I offer water for rinsing the mouth,
and I offer sandal paste for refreshing and anointing the hands
O Supreme Lord Dhanvantari!
Please accept this divine Tāmbūla —
a betel preparation made of sacred areca nut,
fresh betel leaves (nāga-vallī),
and fragrant ingredients like camphor and cardamom.
May it serve as a pleasant refreshment and mouth fragrance for You.
I offer this betel and areca combination to Lord Dhanvantari for mouth refreshment.

O Lord, who reside within the golden womb (Hiraṇyagarbha),
the radiant source of the universe,
and whose brilliance is like blazing gold —
You bestow infinite merit and auspicious fruits.
Grant me peace, health, and serenity.
I offer this symbolic gift of gold, flower, and donation to Lord Dhanvantari with reverence.

ENGLISH MEANING
Whatever sins I may have committed in this or previous births,
may they all be destroyed with every step of this circumambulation.
O Lord, there is no other refuge for me.
You alone are my sole protector.
Therefore, O Compassionate Supreme Lord,
forgive me out of Your infinite grace.

yāni kāni ca pāpāni janma-antara-kṛtāni ca tāni tāni vinaśyanti pradakṣiṇam pade pade anyathā śaraṇam na asti tvameva śaraṇam prabho tasmātkaruṇya-bhāvena kṣamasva parameśvara śrī dhanvantaridevāya pradakṣiṇaṃ samarpayāmi

करकृतं वा कायजं कर्मजं वा

ENGLISH MEANING
O Lord! Any offences I may have committed
through my hands, body, actions, speech, hearing, sight, or mind —
whether known or unknown to me please forgive them all.
Victory, victory to You, O Ocean of Compassion!
All glories to You, O Great Lord, Supreme Healer, and Auspicious One.
I offer this final flower of mantra and devotion to Lord Dhanvantari
Salutations to You, O Lord,
the cause and support of the universe,
the One who acts for the welfare of all beings.
With utmost devotion and effort,
I bow before You with full prostration (ṣaṣṭāṅga namaskāra).
Salutations again and again to You, O Infinite One,
whose divine form has a thousand faces,
a thousand feet, eyes, heads, and arms the all-pervading, all-sustaining Lord.
Now seek pardon from Shri Dhanvantari for any known-unknown mistakes done during Puja while chanting following Mantra.
आवाहनं न जानामि न जानामि विसर्जनम्।

ENGLISH MEANING
I do not know how to properly invoke or dismiss You,
nor do I know the exact procedure of worship forgive me, O Supreme Lord.
My worship may be lacking in mantra, ritual, or devotion yet, O Lord of the Gods,
may it become complete through Your grace.
I commit thousands of mistakes daily, knowingly or unknowingly.
Considering me as Your humble servant, O Supreme Lord, forgive me.
O Lord, grant me auspiciousness, abundant prosperity,
beauty, victory, fame, and protection from enemies.
By remembering or chanting Your name,
even an incomplete act of penance or sacrifice becomes perfect I bow to that imperishable One, Lord Acyuta.
By this worship, performed with whatever materials I could gather,
may Lord Śrī Dhanvantari be pleased.
